You can either fill the form out online and print it, or print the blank form and fill in the information by hand. In either case, sign the completed form and mail the original to:

Expediting the Credit Application Process

To ensure your credit application goes smoothly and as quickly as possible, be sure to follow these tips:

1. Fill out the credit application form completely.

Signatures are required on the credit application. Signatures are also required on the Authorization to Obtain Consumer Credit Report/History form if using personal credit to open a credit account.

2. Provide complete information on concrete suppliers, including fax numbers.

The greater number of references you can supply, the faster your application gets processed, as we can take the first three references that respond. Please list material suppliers only, as leases, credit cards, or services can’t be used as references for our purposes.

3. Click this Customer Questionnaire Document Link to tell us about your project and the products you wish to purchase.
